**You are required to enter a valid credit card number to book your appointment. It will be encrypted and saved securely. You will NOT be charged unless you are in violation of the below policies.**
POLICIES FOR BROW BAR/HAIR REMOVAL/FACIAL SERVICES
CANCELLATION/RESCHEDULE POLICY: 48 hour advance notice is requested to reschedule/cancel, and a 24 hour advance notice is required. If you do not provide at least 24 hour notice to cancel or reschedule, you will be charged half of the treatment cost. In the event of a “no-call/no-show” you will be charged for the entire missed appointment. **I understand that unanticipated events do occur, so please contact me as soon as possible to inform me if you will not be able to make your scheduled appointment.
LATE ARRIVALS: Late arrivals will not be afforded extension of scheduled treatments. Treatments will be rendered only for the remainder of the scheduled appointment and you will be responsible for payment for the full service. If you arrive so late that your treatment cannot be given, it is considered a missed appointment and you will be charged.
NO-CALL/NO SHOWS ARE NOT TOLERATED. IF YOU NO SHOW ANY APPOINTMENT, YOU WILL BE CHARGED FOR YOUR MISSED APPOINTMENT AND ANY/ALL FUTURE APPOINTMENTS WILL BE POLITELY DECLINED.
POLICIES FOR PMU SERVICES
PLEASE READ "ALL YOU NEED TO KNOW" on www.permanentmakeuptx.com to make sure you are a candidate for PMU BEFORE booking your PMU appointment.
BOOKING FEE: A $100 NON-REFUNDABLE booking fee is required to book your PMU appointment (credited toward your service). Please be aware that you will forfeit the fee if you cancel your appointment, no exceptions. An additional $100 fee will be required to make a new appointment.
RESCHEDULING: A MINIMUM OF 48 HOURS ADVANCED NOTICE is required, otherwise you will lose your booking fee - no exceptions. If you choose to NOT reschedule, you will forfeit your booking fee and any future PMU appointments will require another $100 fee to secure your appointment.
LATE ARRIVALS: If you are more than 15 minutes late to your scheduled appointment, your procedure will need to be rescheduled and you will forfeit your booking fee.
NO-CALL/NO-SHOWS ARE NOT TOLERATED: If you "no-call/no-show" your PMU session, you will lose your booking fee, you will be CHARGED 50%-100% OF YOUR SERVICE COST, and ANY/ALL FUTURE APPOINTMENTS WILL BE POLITELY DECLINED.
NO CHILDREN OR ANIMALS ARE ALLOWED AT YOUR APPOINTMENT. If you show up with children or a pet, your appointment will be cancelled and you will forfeit your booking fee.
**IF YOU HAVE PREVIOUS COSMETIC TATTOOING, YOU MUST GET MY APPROVAL BEFORE BOOKING!! IF YOU SHOW UP TO YOUR APPOINTMENT WITH A BROW/EYELINER TATTOO THAT HAS NOT BEEN APPROVED, I HAVE THE RIGHT TO TURN YOU AWAY AND YOU WILL FORFEIT YOUR BOOKING FEE**
